How online TRT works (and the DEA rule that governs it)
Online TRT follows the same five steps at every major US provider; price and inclusion scope are what differ.
The standard flow
- Intake quiz and account. Symptoms, history, goals, about ten minutes.
- Bloodwork. At-home draw kit (Hone's included model) or a Labcorp/Quest requisition.
- Physician review. A licensed doctor or NP reviews labs and intake; some providers use video, most use messaging.
- Protocol shipped. Testosterone cypionate (most common) plus supplies, monthly or per-4-weeks.
- Ongoing monitoring. Repeat labs at about 90 days, then semi-annual at most providers.
The legal frame
Testosterone is a Schedule III controlled substance. Online prescribing operates under DEA telemedicine flexibilities, extended through Dec 31, 2026. State boards add a second layer: multi-state-licensed providers (PeterMD advertises itself as North America's largest online clinic) cover more states than smaller operators, and some providers exclude New York and other strict-prescribing states. A provider that quotes without asking your state is a red flag.
What telehealth does not replace
In-person chains (Ageless at roughly 50 locations, LowT Center across Texas) run their own labs and injections. Insurance-covered endocrinology remains available for documented hypogonadism. Fertility planning (hCG, sperm banking) is a pre-start conversation everywhere.
